Sophisticated Cranial Surgery at the International Hospital of Bahrain
Recently, an ultramajor, sophisticated cranial surgery was performed to a male Bahraini patient aged 51 years who arrived to the ER department of the International Hospital of Bahrain in a state of coma and severe irritability with left sided hemiplegia and right upper motor neurone facial palsy.
An urgent MRI on the brain was done and this revealed the presence of a large right sphenoidal ridge meinigioma at the base of the skull, surrounded by massive peri-focal oedema causing marked compression on the brain tissue with shift of midline stuctures and subfalcine herniation.
Dr. Hassan Moukhtar, the neurosurgery consultant at the International Hospital of Bahrain, decided to go immediately for an urgent craniotomy to excise the tumor and save the life of the patient. At surgery, the tumor was found to be large ( 6 x 7 x 3 cm ) fleshy in its consistency, grayish-red in colour and highly vascular. It was abbuting the greater wing of the right sphenoid bone at the skull base and adherent to the main trunk of the right middle cerebral artery, which is the main blood supply of the right cerebral cortex, and the right cavernous sinus. Also, the tumor was found to compress the vital areas of the brain, mainly the motor area and the brain stem, by causing subfalcine and transtentorial herniation from the massive peri-focal oedema.
After operation, the patient was put on artificial hyperventilation for 48 hours then started to be weaned from the ventilator gradually until he regained his full conscious level and became able to talk and react normally to external stimuli. The histopathological examination of the excised brain tumor confirmed the preoperative provisional diagnosis of sphenoidal ridge meningioma. The patient was put on an intense rehabilitation program and was discharged from the hospital on the tenth post operative day in a fully conscious state and also neurologically intact to return back to his home and family to start a new life.
After three months from the operative procedure a follow up MRI on the brain was done for the patient and this revealed complete excision of the tumor without any evidence of tumor residual or recurrence.
